I voluntarily apply to become a member of ASCO where membership is selected. I confirm that I have read and understood the objectives, values and Code of Conduct of the organisation. I agree to abide by ASCO's Constitution, policies and lawful decisions and to uphold justice, equality, integrity, accountability, African solidarity and human dignity.

I understand that membership may be suspended or terminated where there is serious misconduct or conduct that brings ASCO into disrepute, in accordance with ASCO's Constitution and disciplinary procedures. I confirm that all information provided is true and correct.
